Community based support

We enable people to have access to their community and form meaningful relationships, reducing social isolation and improving overall well-being.

Our team aims to manage or improve the mental health of the people we support by working alongside social workers and community mental health teams. We provide support to meet the unique needs and goals of each person supported.

The service provides support for a variety of needs, including:

  • Housing Support: Helping individuals maintain their housing by offering guidance on tenancy management, resolving housing issues, managing their home and ensuring they can stay in their homes.
  • Access to Services: Supporting individuals in accessing mental health services, benefits, social care, and other relevant support networks.
  • Personal Wellbeing: Offering advice and assistance to improve overall well-being, such as developing coping strategies, improving daily routines, or managing mental health symptoms.
  • Life Skills Development: Helping people build essential skills such as budgeting, cooking, cleaning, and other tasks that improve independence and self-sufficiency.
  • Advocacy and Empowerment: Providing advocacy services to ensure the voice of the individual is heard in healthcare, legal, or social situations, and promoting empowerment through decision-making.

Floating support means the service is not tied to a particular building or location. Support is provided in the person’s home or community environment, and the amount of support can be adjusted based on the person’s needs.

We provide a flexible, short- or long-term service tailored to each individual, which enables those with mental health challenges to live more independently and with greater confidence with an emphasis on health and wellbeing.
